Company Meeting Absentee Notification Form

This form is provided to simplify the process of notifying Company administrative Officers of your status of not attending the Monthly General Meeting.  It is your duty and responsibility to keep the Company Leadership informed of your status and attendance of Company Meetings and Events.

All Members are required to attend at  least six (6) meetings a year to maintain active status in good standing with the Company.
