MCVFD Application Information and Requirements:

Once your application is reviewed by the membership committee you will be
contacted by telephone, e-mail, or both to arrange for a personal (face to face)
interview at the fire station. At your interview please be prepared to provide
documentation (i.e. driver's license, copies of certificates of training, etc), and to
answer questions about your membership request (i.e. why you'd like to join, what
you're hoping to get out of membership, etc.) We look forward to getting to know and
working with you!

All fields marked with an asterisk (*) on the application form denote mandatory fields;
you MUST fill them in order to submit the form.

***Please note: There is a $10 application fee due at the time of your scheduled
interview.

By submitting the application to us, you hereby declare that all statements and
information on the application is true and complete, and understand that any
untruth, misleading answer, or deliberate omission or concealment may be grounds
for refusing membership with the Maryland City Volunteer Fire & Rescue Squad, Inc.

You also hereby authorize the Maryland City Volunteer Fire Dept. & Rescue Squad,
the Anne Arundel County Fire Dept., or their respective agents/representatives to
communicate with any individual or organization listed on this application for the
purpose of investigating or confirming your character, conduct or record, and to
keep and preserve records of such communications. Additionally, you release all
parties from all liability for damage that may result from furnishing any information.

If accepted for membership, you agree to read and comply with all company and
county fire department rules and regulations, and policies. You agree that upon
termination of your membership with the Maryland City Volunteer Fire Department &
Rescue Squad that you will return all company and/or county property in your
possession.

THE MARYLAND CITY VOLUNTEER FIRE DEPARTMENT & RESCUE SQUAD, INC.
CONSIDERS MEMBERSHIP APPLICATIONS WITHOUT REGARD TO RACE, COLOR,
RELIGION, SEX, NATIONAL ORIGIN, OR TO THE EXTENT PROVIDED BY LAW, AGE,
MARITAL STATUS, PREGNANCY, MEDICAL CONDITION, DISABILITY OR ANY
PROTECTED CLASS AS DEFINED BY FEDERAL, STATE, OR LOCAL STATUTE.
